The course is highly interactive and uses actors and workbooks to both enhance and support the learning style of the delegates |
The half day workshop and supplementary materials teach delegates how to:
• set up a self care management programme
• identify the right patients for the programme
• implement and enable a quality service
• perform data extraction*

The OwnHealth® Choice disease and condition modules provide university accredited distance learning training (via CD-ROM) on clinical guidelines within key disease areas (such as National Institute for Health and Clinical Excellence guidelines), a comprehensive training manual to support practice staff to understand the disease area alongside a care manager guide and 50 comprehensive care books for patients.
Disease modules available include diabetes, COPD, CVD, HF, weight maintenance, chronic kidney disease, stroke and transient ischaemic attack, hypertension and the at risk elderly |
