Supporting individuals to take control
Support
Specially trained nurses, known as 'Care Managers', work with people with long term conditions who meet agreed selection criteria and have been nominated by their GP, to develop personal action plans. These plans reflect their individual circumstances and personal goals whilst helping them to understand their medical conditions.
Empower
Members of the programme gradually take on more responsibility for their own care through better understanding of their condition.
Change
In time members ‘graduate’ from the programme allowing both capacity for new members but; importantly to that individual member, they are recognised as having the confidence and knowledge to manage their own condition.
