A NEW APPROACH

OwnHealth^® is a new approach to the management of long term conditions. It brings together the experience of Pfizer Health Solutions, the skills of specially trained care managers and proven decision-support software, to provide individuals with long term conditions a personalised service, delivered in partnership with their local NHS team.

The goal of OwnHealth^® is to support individuals to make changes in their daily behaviour that will have a positive impact on their health and as a result will encourage more appropriate use of local NHS services.

THE FUNDAMENTALS OF OWNHEALTH^®

Care managers are pivotal to the success of OwnHealth^®. They are fully trained and highly experienced nurses who are able to give self-care guidance to individuals over the telephone.

The care manager is equipped with software that guides them through the eight care management priorities when talking to an individual. There are specific prompts for different long term conditions. Not only will the care manager call the individual at pre-defined times, but also the GP can encourage individuals with long term conditions to call their care manager for extra support and advice on those occasions when a visit to their GP is not essential.

The relationship the care managers develop with the local primary care teams is also vital in ensuring coordinated support for those on the programme. Care managers are there to build on the professional care provided and to enhance the relationship each person has with their primary care team.
