Pfizer Health Solutions will provide full support and work closely with the care managers to optimise the impact of OwnHealth^®. We will offer a tailored service according to the specific needs of each organisation. We will lead project implementation and management working with you through all stages including reviewing the project outcomes and conducting a full evaluation. OwnHealth^® will set out success measures from the outset which will include evaluation of:

People with long term conditions

• clinical outcomes
• behavioural outcomes (ie healthier behaviour by participating individuals)
• psychosocial outcomes (eg improved quality of life.)

The NHS and Clinicians

• the programme’s influence on the effectiveness of patient management
• stakeholder opinions on the value of the programme
• impact on use of services.

Your Role

We are looking for partners who are committed to innovation in personalised care, who are prepared to dedicate the time to project management and share our enthusiasm for ensuring outcomes and progress on an ongoing basis. More specifically, this will include patient identification, stakeholder engagement and joint promotion of the project.
